Hepatitis C-related cirrhosis is currently the most common cause of orthotopic liver transplantation worldwide. However, hepatitis C virus infection invariably recurs after liver transplantation. In recent years, it has become apparent that hepatitis C recurrence progresses more rapidly, and is less responsive to antiviral treatment, compared to pre-transplant infection. The present review addresses the natural history of hepatitis C recurrence in the liver transplant recipient, the factors implicated in its progression, and the rationale for prevention and antiviral treatment.
