A 70 year old man with a metastasizing bronchogenic adenocarcinoma inside a pituitary adenoma is presented. Preoperative MRI findings of the lesion were suggestive in view of the peculiar dishomogeneous appearance of the tumor. A review of the literature revealed only ten previous cases to which we add our personal one. The salient clinico-pathological and diagnostic features of this rare lesion are discussed in the light of the available literature
