Advances in the basic scientific research within the field of computer assisted oral and maxillofacial surgery have enabled us to introduce features of these techniques into routine clinical practice. In order to simulate complex surgery with the aid of a computer, the diagnostic image data and especially various imaging modalities including computer tomography (CT), magnetic resonance imaging (MRI) and Ultrasound (US) must be arranged in relation to each other, thus enabling a rapid switching between the various modalities as well as the viewing of superimposed images. Segmenting techniques for the reconstruction of three-dimensional representations of soft and hard tissues are required. We must develop ergonomic and user friendly interactive methods for the surgeon, thus allowing for a precise and fast entry of the planned surgical procedure in the planning and simulation phase. During the surgical phase, instrument navigation tools offer the surgeon interactive support through operation guidance and control of potential dangers. The objective of the present study was to observe the effect of the position of the mandible on the accuracy of the cross-sectional images obtained by reformatting the Computerized-Tomography (CT) scan. Additional aim is to evaluate the ability of an original software (DentalVox-Erascientific) to reconstruct accurately and without distorsion such measurements on the reformatted images, regardless of the positioning of the mandible. The DentalVox software (Erascientific) allowed to reconstruct cross-sectional images with distorsion due to initial mandibular positioning close to zero. Many applications are still being developed or are still in the form of a prototype. It is already clear, however, that developments in this area will have a considerable effect on a surgeon’s routine work.

La Tomografia Assiale Computerizzata (TAC) e la Risonanza Magnetica Nucleare (RMN) sono le modalità adottate in medicina per ottenere immagini relative a sezioni del corpo umano. Nell’ultimo decennio la TAC è diventata lo strumento più diffuso per la valutazione della struttura ossea su cui effettuare il posizionamento di impianti dentali ). A tale scopo sono stati realizzati strumenti software che consentono un’elaborazione delle immagini ottenute con la TAC al fine di ottenere una diagnosi pre-operatoria corretta e una pianificazione dell’installazione dell’impianto . In lavori precedenti è stato dimostrato che le immagini TAC sono affette da una distorsione che varia dallo zero al sei percento. Questa distorsione può essere dovuta all’allineamento del paziente durante la scansione TAC, ai suoi movimenti e, in ultimo, alla saturazione dei pixel che costituiscono l’immagine. In questo lavoro presentiamo un sistema software che consente una valutazione corretta della strutturaossea sottostante un impianto da realizzare e delle sue misure anche da una sola acquisizione TAC. Il sistema, denominato DentalVox, applica tecniche di elaborazione di immagini e in particolare algoritmi di interpolazione alle immagini TAC originali per produrre nuove immagini che possono appartenere a piani orientati diversamente rispetto a quello dell’acquisizione originale. Tutto ciò consente di non esporre il paziente a eccessive radiazioni ottenendo, al contempo, migliori risultati meccanici ed estetici nella realizzazione di protesi da installare nella struttura ossea.
