Intramural reduplication cyst of the esophagus is unusual. Esophageal cyst may be considered a failure in the vacuolation process from the primitive solid tube. We present a case of intramural esophageal cyst found in a 66 years old mild-symptomatic woman. Although symptoms and barium swallow or CT scan suggested a leiomyoma, the cyst was detected by means of EUS (Endoscopic Ultrasonography). This diagnostic technique permitted to clarify the cystic nature of the lesion and its anatomical relationship with esophagus and neighbouring structures. The patient was successfully treated by surgical excision of the cyst by a right-sided thoracotomy, she recovered completely and was discharged on the tenth postoperative day.
