We appreciate the comments from Weber et al1 on our article published in Anesthesia & Analgesia on the association of severe intraop erative hyperglycemia (SIH) and infectious complica tions after elective brain neurosurgical procedures.2 In their letter, several important concerns are presented, including: criteria used for the diagnosis and selection of postoperative infections, blood glucose concentration (BGC) thresholds (severe, moderate, and mild hypergly cemia), perioperative glucose control (criteria for start ing an insulin infusion and treatment protocol), and influence of confounding factors tested in our study.
