Scintigraphy with Tc-99m HMPAO-labeled leukocytes proved useful to diagnose intestinal lesions in Crohn disease (CD). The hybrid device combining single photon emission tomography (SPECT) and computed tomography (CT) has been reported to improve SPECT diagnostic accuracy matching functional and anatomic information. The authors describe a 30-year-old man with ileorectal anastomosis for CD. In this case, SPECT shows an area of leukocyte accumulation within the pelvis, and SPECT/CT precisely localizes this uptake along a perianal fistulous tract.
