Insulin receptor (INSR) and insulin-like growth factor I receptor (IGF1R) transduce the metabolic and growth-promoting actions of insulin and IGF1;. INSR mutations give rise to hyperinsulinism and hyperandrogenism, while IGF1R mutations cause intrauterine growth retardation with reduced birth size and short stature. Mature insulin and IGF1 receptors (INSR, IGF1R) are constituted by an alfa and a beta subunit. INSR and IGF1R intracellular beta subunits share about 80% of aminoacid sequence; extracellular alfa subunits are more diverse. INSR and IGF1R can form heterodimers hybrids of unknown physiological function that bind IGF1 with much higher affinity than insulin. Thus, it seems likely that hybrids mainly mediate IGF1 function. We describe two sisters with hyperinsulinism, hyperandrogenism, low birth weight, short stature and reduced adipose tissue who carry heterozygous mutations of INSR and IGF1R.. IGF1R mutation impairs processing of pro-receptor to mature receptor, while dominant-negative INSR mutation abolishes transphosphorylation of INSR/IGF1R hybrids. We studied ex vivo the effects of these mutations in patients’ fibroblast and also IGF1R biosynthesis and hybrids function in vitro. GFP-FOXO1 transfected in fibroblast of normal sister localized mainly in the cytoplasm and became more nuclear upon FCS starvation for 16 h. No effect was observed after FCS starvation of fibroblast of the two affected sisters, with GFP-FOXO1 remaining mostly nuclear even after incubation with high dose insulin+IGF1. RNA micro-array and real-time PCR analysis revealed that IGF2, along with other genes (LHX8), was underexpressed in fibroblasts of the affected sisters as compared to the younger sister and the father; these genes are implicated in intrauterine growth and ovarian function; INSR and IGF1R expression was normal. iPSC have been derived from this kindred to the aim of gaining insight into the (patho)physiopathologic role INSR/IGFR hybrids in adipose and possibly bone development/function.

Il recettore dell'insulina (INSR) e il recettore del fattore di crescita insulino-simile (IGF1R) sono coinvolti nella trasduzione del segnale metabolico e di crescita promosso dall’azione dell’ insulina e di IGF1. Mutazioni nel gene INSR provocano iperinsulinismo e iperandrogenismo, mentre le mutazioni nel gene IGF1R causano ritardo di crescita intrauterino, ridotta dimensione alla nascita e bassa statura. INSR e IGF1R sono costituiti da una subunità alfa e una subunità beta. La subunità beta del recettore dell’ insulina, porzione intracellulare, ha circa l’ 80% di omologia della sequenza aminoacidica con IGF1R; mentre la subunità alfa, extracellulare, è più diversificata tra i due recettori. INSR e IGF1R possono formare ibridi con funzione fisiologica sconosciuta che si legano ad IGF1 con affinità molto più alta rispetto all’ insulina; ciò fa supporre che gli ibridi mediano prevalentemente la funzione di IGF1. In questo lavoro descriviamo due sorelle con iperinsulinismo, iperandrogenismo, basso peso alla nascita, bassa statura e con riduzione del tessuto adiposo che presentano mutazioni eterozigoti di INSR e IGF1R . La mutazione in IGF1R danneggia l'elaborazione del pro-recettore a recettore maturo, mentre la mutazione dominante negativa del gene INSR abolisce la transfosforilazione degli ibridi INSR/IGF1R. Abbiamo studiato ex vivo gli effetti di queste mutazioni nei fibroblasti dei pazienti affetti, la biosintesi di IGF1R e la funzione degli ibridi in vitro. I fibroblasti della sorella normale trasfettati con il plasmide GFP-FOXO1 localizzano il fattore di trascrizione principalmente nel citoplasma, localizzazione che diviene più nucleare se le cellule vengono private di siero bovino per 16 ore. Nessun effetto è stato osservato dopo la starvation nei fibroblasti delle due sorelle mutate, GFP-FOXO1 rimane per lo più nucleare anche dopo stimolazione con alte dosi di insulina e IGF1. L’analisi di RNA micro-array e real time PCR hanno rivelato che il gene di IGF2, insieme ad altri geni (LHX8), è sottoespresso nei fibroblasti delle sorelle affette rispetto alla sorella minore (sana) e il padre (presenta esclusivamente la mutazione in INSR). Questi geni sono coinvolti durante la crescita intrauterina e funzione ovarica mentre l espressione di INSR ed IGF1R è normale. Sono state generate iPSC da questi affetti con scopo di approfondire la conoscenza sulla fisiopatologia del ruolo degli ibridi INSR / IGFR nel tessuto adiposo ed, eventualmente, nello sviluppo delle ossa.
