A 51-year-old man was examined for a suspected tumor of the foramen magnum. A computed tomographic (CT) scan confirmed the presence of a huge mass, which was thought preoperatively to be a neurinoma. At operation, this was revealed to be a giant thrombosed aneurysm of the vertebrobasilar junction that had eluded preoperative angiographic examination. The CT features of this atypical case are discussed
