We studied 138 patients with large-vessel stroke and 61 patients with cardioembolic stroke to determine the association between infection with Helicobacter pylori and ischemic stroke. After correcting for traditional risk factors, we found an association between cytotoxin-associated gene-A (CagA)-positive strains of H pylori and patients with large-vessel Stroke. No association was found for CagA-negative strains. Neither CagA-positive nor CagA-negative strains showed any positive association with cardioembolic stroke. Infection with CagA-positive H pylori strains may represent an additional risk factor for ischemic stroke due to atherosclerosis.
