Labeled Problem. We report here the case of a 30 years old woman affected since childhood with hypereosinophilia and with APS (aPL + RSA). She presented to our attention a short time before the beginning of a (twin) pregnancy. All the clinical and laboratory data confirmed the diagnosis of hypereosinophilia syndrome and of APS. A therapeutic trial employing High Dose intravenous Immunoglobulins (IVIg) was started according to already reported criteria. Eosinophils and different cytokines were studied during pregnancy. Both angioedema and urticaria completely disappeared starting from the day after the very first infusion of IVIg. Eosinophils decreased throughout IVIg treatment. Pregnancy had a normal course. Caesarean section was performed at 37(th) week of pregnancy allowing the birth of two healthy babies. IVIg therapy was useful in the conditions described (hypereosinophilia, angioedema, APS, pregnancy). The reversion of hypereosinophilia was completely persistent even two years later.
